Developmental Screening Tools for Health Care Providers
A new web page for pediatric health care providers has been added to the CDC Child Development Website. The new page provides tips for primary care practitioners on integrating developmental screening into their practices efficiently, at low cost, and while ensuring coordination of care. A sample flowchart of clinical screening activities and an example of how physician and non-physician staff roles may be delineated to make routine screening feasible are provided as well (these can be printed from the website).
Pennsylvania Medicaid Adult HIV Clinical Practice Guidelines
The guideline was developed to help practitioners care for adult Medical Assistance recipients with HIV/AIDS. It was created using currently existing national protocols and is intended to be broad enough to cover all adult members living with HIV/AIDS and allow for accepted variations in clinical treatment and pharmaceutical utilization.

2009 Adult Preventive Care Guidelines
We are proud to announce the availability of the Adult Preventive Care Flow Sheet. This form is based upon the ACCESS Plus Adult Preventive Care Guidelines and is a great reminder “check off” list for providers when completing an adult preventive exam. This simple tool can be included in the patients chart for a quick reminder of needed services. The form is easily reproducible and may be utilized as template for the electronic medical record.

Transition Health Care Checklist
The Pennsylvania Department of Health Southwest Regional staff and their community partners recognized a gap in health services for youth with special health care needs. When these youth leave school, they transition from having different or no insurance coverage, from seeing pediatric specialists to searching for adult medical specialists, and from good coverage for medications to having to understand new systems for obtaining medicines. Working with the State Transition Team, agencies, and families, the Transition Health Care Checklist was created to guide youth, families, and professionals during this time of change.
