The Assessment and Triage of Suicidal Adolescents Survey

Increasingly, adolescents turn to primary care providers for assistance with mental health problems. Yet, screening/triage takes time, and resources, referral options and payment are limited. These barriers and challenges are especially frustrating when youth present with suicidal thoughts or behaviors.

Use the link below to share your experience of working with adolescents presenting with mental health and suicidal problems in your practice. Through this survey you can give voice to the concerns, needs, and strengths of the primary care community related to adolescent behavioral health. The survey takes no more than 15 minutes and will guide continuing education programs and practice supports and support lobbying efforts with the state and insurance companies who could better assist us with this growing public health problem. We appreciate your effort on this project and will provide feedback through your state professional organization.

The survey is supported by the federal Garrett Lee Smith Suicide Prevention in Primary Care Program and managed by the PA Dept. of Public Welfare. This project provides staff training, a web-based screening tool, and assistance to strengthen linkages to mental health providers. BY COMPLETING THIS SURVEY, YOU GIVE US PERMISSION TO USE YOUR RESPONSES IN A RESEARCH PROJECT. This survey is part of a research study being conducted at the Children's Hospital of Philadelphia. Your participation is fully voluntary. The survey is anonymous and will not collect any information which could identify you. Data will be aggregated and only examined as a group. No one will be able to link this data back to you as an individual. Thus, there is minimal risk of loss of confidentiality if you agree to participate. In addition, the benefits to the larger primary care community could be important. The study data will be used to inform policy makers and payers about the needs for more support for mental health issues in primary care. Your alternative to completing this survey is to not complete the survey. If you decide not to participate or if you change your mind, there will be no penalties or loss of benefits to which you are otherwise entitled.
